How to apply for a social service
This general procedure is roughly applicable to all social services. Some may require extra documents or assessments – you can find information on the service’s website or by contacting the municipal or municipal office/private provider.
Procedure:
The local authority/municipality will receive your application and decide on its allocation based on your health and social situation.
The form can be found on the website of your local authority/municipality.
If you pay for the service yourself up to at least the amount of the economically justified costs, you do not need an assessment.
IMPORTANT:
If a person is unable to sign the application due to his/her health condition, another person may sign the application based on the attending physician’s certification of the adverse health condition.
The application must be submitted by post, electronically or in person to the office of the competent authority in your place of residence and be accompanied by the necessary documents.

In any case, you will need:
- a copy of a valid ID card or passport,
- a report on the current state of health, not older than 6 months (specialist medical report, hospital discharge report, etc.) and, for specialised facilities, proof of a psychiatric or neurological examination ,
- a copy of the court’s final judgement of disqualification (if any),
- if the application is made by a foreigner, a residence permit from the competent authority and a certificate from the employer or school.
The doctor, nurse and social worker assess the situation and the authority issues a decision on dependence on social services with an assessment of dependence, including the extent of assistance needed and the degree of dependence on the assistance of others.
The application must be processed by the Office within 30 days, or exceptionally within 60 days, and the decision will take effect within 15 days of receipt. In urgent situations (in case of serious threat to life or health), the service may be started without waiting for a decision. The decision with the assessment will be delivered subsequently.
The decision shall be delivered by post for collection in the applicant’s own hands.
Once you have received a decision on dependency and an assessment for a social service, you must write to the authority that issued the decision. The application must state the name of the social service provider chosen by the applicant and the place where the social service is provided.
We encourage you to visit the facilities and speak to the staff responsible for admissions.
You can ask the office for a list of all the facilities in the area that provide the service you need. These are:
- facilities of municipalities, towns and LDCs = public providers,
- religious and civic organisation facilities = non-public providers.
Attention:
If you need placement in a facility on a truly urgent basis, you can request immediate service without enrolling in a waiting list.
If you do not want to be placed in a large establishment, you have the right to apply for a family-type establishment (with fewer than 40 places).
If there is a vacancy with a non-public provider, you can only sign the Service Contract after confirmation from the authority that it will co-fund the service. In this case, you will pay only part of the full feethat is not paid by the municipality or the municipal district
By post, in person or electronically to the relevant address:
- city/municipality – if it is a day-care centre, a facility for the elderly or a care facility,
- Local authority – if it is a supported housing facility, rehabilitation centre, social services home or specialised facility,
- directly to the establishment, if the founder is a city/municipality, VUC.
Together with the Application for the provision of social services, please deliver a copy of the final decision on the need for social services.

What to look out for when contracting a social service provider?
We recommend you clearly check, what services are included in the price, or what amount is extra.
The law determines which activities are paid for and which are provided free of charge. The price for the same type and the same range of activities within this service must be the same for all clients of a given provider; and the method of its calculation must be publicly accessible and comprehensiblefor applicants and recipients of social services. This does not mean that all citizens pay the same fee, as it also depends on the level of income and assets of the citizen. Prices may change in line with pension increases, but also increases in reimbursement by the municipality/city
and the municipality or non-public provider. Inflation, energy and food prices, and changes in wage levels also have an impact on reimbursement rates.
For certain other activities, such as medical rehabilitation, art therapy, you have to pay for yourself and enter into a separate contract with the provider to provide them.
For some facilities and services, you can choose to upgrade – for example, to exclusive room amenities, a larger room and a higher frequency of cleaning. There is usually an extra charge for upgrades, and its details must be included in the contract you sign.
Attention!
The social service provider may not make the conclusion of the contract conditional on further payment or non-monetary consideration.
