- have reached retirement age and need the provision of social services in this facility for other serious personal reasons.
Attention!
The facility for the elderly is the only facility where seniors who are not dependent on the help of others but have other serious reasons for admission (illness, etc.) can go.

Specialised equipment

When am I entitled to the services of a specialist facility?
Only people with level V dependency who suffer from a specific diagnosis are admitted to the specialised facility.
Most often they specialize in one of the following diseases:
- Parkinson’s disease,
- Alzheimer’s disease,
- Pervasive developmental disorder (disturbed psychological development),
- Multiple sclerosis (long-term disease),
- Schizophrenia (isolates a person from reality),
- Dementia of various types of etiology,
- Deafblindness (dual sensory loss),
- AIDS,
- Organic psychosyndrome of severe degree (aggressive and dangerous action on oneself and on one’s surroundings).
If you need the help of such a facility, it’s best to find a specialized facility in your area that focuses on your exact diagnosis. The staff are trained to help you as effectively as possible, and the whole facility is adapted to living with the illness/disability (wheelchair access, etc.).
How do I request the services of a specialist facility?

Rehabilitation centre

When am I entitled to the services of a rehabilitation centre?
The rehabilitation centre cares for people with:
- low vision,
- hearing impairment,
- severe hearing loss,
- or complete deafness.
You can use the services of the rehabilitation centre 3 months, with a possible extension for another 3 months if necessary.

When am I eligible for day centre services?
The day centre provides social counselling and leisure activities for people who:
- have reached retirement age,
- have a severe disability or adverse health condition
- are grandparents and live with a grandchild or granddaughter.
How do I apply for a day centre?
If you are interested in the possibility of using social services in a day centre, it is necessary to contact the day centre directly or your city/municipality, which will help you to choose and integrate into the day centre.

When do I qualify for day care?
The retirement home is intended for pensioners who are dependent on the help of other people with at least level III dependency.
The degree of dependency is calculated on the basis of several parameters from the social (within the competence of the city/municipality) and medical (within the competence of the doctor) assessment. Supported living facility
You can think of it as living in a separate “apartment” or in a larger group with more people. Here you live independently under the supervision of a trained professional who will help you if you need it.
Supported living facilities normally provide:
- Supervision,
- accommodation,
- social counselling,
- assistance in exercising rights and protecting interests,
- conditions for food preparation, cleaning, laundry, ironing and maintenance of linen and clothes,
- social rehabilitation (promoting independence, self-sufficiency and independence in basic social activities).
This service is not provided to citizens who receive an old-age pension, but is provided to citizens of pre-retirement age and also to disabled pensioners
When am I entitled to accommodation in supported housing?
Anyone can apply for supported housing if:
- you are at least 16 years old and under retirement age
- is dependent on the help of another person, but can function independently in most cases.

Social Services Home (DSS)
People who require 24-hour care can be accommodated in the facility, but outpatient care is also possible – on-site assistance followed by a trip back home.
The social services home normally provides:

- accommodation,

- cleaning, laundry, ironing and maintenance of linen and clothes,
- personal equipment,
- developing creativity and work skills,
- leisure activities,
- nursing care,
- conditions for safekeeping of valuables.
This service is not provided to citizens who receive an old-age pension, but is provided to citizens of pre-retirement age and also to disabled pensioners.

A social services home usually accommodates people who:
- are dependent on the help of other natural persons at least at the V. degree of dependence,
- are blind or partially sighted and their dependency is at least level III
