Card of a severely disabled person (PWD)
With this card you can apply for various discounts and benefits (at cultural events, offices, means of transport, etc.).

Who is entitled to a disability card?
To get a card, you need a valid decision showing that you are severely disabled (e.g. a decision on a cash allowance from the local authority).Disability card is for people with severe disabilities. If the person also needs to be accompanied or assisted by an assistance dog, they get a disabled person’s card with a guide.
How do I apply for a disability card?

Completing the application
Pick up and fill in an application form for a Severe Disability Card at the Labour, Social Affairs and Family Office in your place of residence. If you need help filling it in, be sure to ask the staff at the office, who will be happy to help you.
You can also download the application from the website MLSA and fill it out in advance.

Medical finding
You can get a medical report form from the Office of the Social Security Administration. This form is filled in by your doctor, and will be completed by any findings from specialist doctors.

Submit an application
You must submit your application, together with the medical report, to the Office of Social Security and Social Affairs in your place of residence. You can also submit your application electronically, verified by an electronic signature.

Issue of an opinion
The office has 30 days to issue an opinion and process your application. If the assessing doctor determines that your level of impairment exceeds 50% functional impairment, you will be entitled to a disability card.
