When do I qualify for a transport service?
They can use the transport service:
- persons with severe disabilities who need individual transport by private motor vehicle,
- you have an adverse health condition, limited mobility on flat surfaces or stairs, or impaired orientation.
Referral to the transport service must be proven by an assessment issued by the competent Labour, Social Affairs and Family Office on the basis of a medical assessment.
How do I start using a transport service?
If you are interested in a transport service, you must contact the city/municipality where you live. After checking the certificate of need for transportation service, you have the right to call the transportation service for a transfer:
- to a medical facility,
- to a social facility,
- to public institutions (city hall, etc.),
- for a cultural event,
- on the train,
- for other occasions.
Attention,
transport services usually operate only during dedicated days and hours. Therefore, it is a good idea to pre-book a transport service by phone and check the availability of the vehicle.
How much does the shipping service cost?
The price of the transport service may vary from municipality to municipality.
In most cases you will pay a flat fee for using the service, and an additional price for each kilometre or hour of waiting.
